• Take time to understand what matters most to you now, and what your needs may be in the future;
• Help you find and connect with the right supports and services;
• Offer guidance on questions you may wish to ask your healthcare providers;
• Connect you with compassionate anticipatory grief support;
• Answer your questions about hospice care—what it is, when it may be helpful, and how to access it.
