Position Description:

The Registered Practical Nurse (RPN) at St. Joseph’s Hospice plays a vital role in delivering comprehensive, person- and family-centered hospice palliative care. This position offers an opportunity to work in a supportive, home-like environment where evidence-based practice, interdisciplinary collaboration, and emotional presence are the cornerstones of care. Our nurses are key contributors to a culture that supports meaning, connection, and comfort during one of life’s most profound transitions.Working as part of an interdisciplinary team, the RPN provides expert clinical care while addressing the multidimensional needs of residents and their families, in alignment with the eight domains of care outlined in the Model to Guide Hospice Palliative Care (Canadian Hospice Palliative Care Association) and the ethical principles laid out in the Health Ethics Guide (Catholic Alliance of Canada).

Qualifications:
  • Current registration and in good standing with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse
  • Current certification in CPR/BLS
  • Certification with the Canadian Nurses Association (CNA) in palliative care or equivalent experience in palliative, hospice, or end-of-life care preferred
  • Fundamentals of Hospice Palliative Care or CAPCE of Certificate completion, or commitment to obtain within 6 months of hire
  • Regular, on-site attendance is a requirement of this position
Skills, Abilities and Experience:
  • Demonstrated knowledge of palliative care principles and symptom management across all eight domains of care
  • Strong interpersonal, communication, and emotional intelligence skills•Ability to practice relational, tauma-informed, and culturally safe care
  • Comfort engaging in goals-of-care conversations and supporting families through anticipatory grief
  • Commitment to ongoing professional learning and reflective practice
  • Demonstrated strong teamwork skills and consistently treats others with professionalism and respect.
  • Experience in hospice, palliative, or community-based care settings preferred
  • Experience with electronic charting/EMRs, documentation, and MS Office preferred
Principal Duties:
  • Provides holistic and individualized nursing care aligned with resident goals and values
  • Collaborates with interprofessional team, residents, and caregivers in the development, implementation, and evaluation of care plans
  • Supports residents and families with emotional, spiritual, and practical needs•Administers medications and interventions for symptom control and comfort
  • Engages in effective and respectful communication with interdisciplinary team members, physicians, and external providers
  • Participates in quality improvement, ethics discussions, and professional development•Supports continuity of care through active listening, advocacy, and presence•Serves as a mentor and role model for students and new team members•Other related duties as assigned
